Insurance
If you have a PPO plan with out of network coverage, services may be covered in full or in part by your health insurance or employee benefit plan. HMO insurance will not provide coverage for services provided at this facility. Please call and check your insurance coverage carefully by asking your insurance provider the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Payment
Payment is due in full at time of service. A super bill can be provided to you, for you to submit directly to your insurance carrier. If your insurance covers services for out patient mental health, they can reimburse you directly, per your particular insurance plan.
Cash, check and all major credit cards are accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
